You have my permission to email me class related information and announcements. I understand that my personal information will not be released to any third party, and is for the exclusive use of communication from Kindermusik with Maria. You have my permission to use photographs or video of myself or my child taken at Kindermusik with Maria classes, lessons, or special events. If paying by credit or debit card I hereby authorize Kindermusik with Maria to charge tuition payments for each month I am attending classes. I understand that written or email notice must be given to the Kindermusik with Maria office prior to the 1st of the month if I need to withdraw from the program and cancel my enrollment. A NSF fee of $20 will be assessed for any declined electronic funds or late payments. With this enrollment, I release any and all rights and claims for damages against Kindermusik with Maria and its Staff in the unlikely event of injury sustained by myself or my child(ren) during the course of or as a result of my enrollment at Kindermusik with Maria.
